Job description

Reporting to the Chief Executive Officer, you will:

  • To oversee the administration and operations of the Social Service Department.
  • Provide regular updates to next of kins (NOKs) on the beneficiaries and families.
  • Support and present cases to the Admission & Review Committee for evaluation and approval for admission to the Home.
  • Case Manager for the Individual Care Plan (ICP) of residents, clients and Training in Independent Living (TIL) residents.
  • Coordinating admissions & discharge, means testing exercise, coordinate and schedule ICP conferences with beneficiaries and their families/caregivers.
  • Liaise with MSWs and/or Doctors or referring agency for relevant medical information especially on present treatment if these have not been given in the application form.
  • Conduct regular groupwork with residents and multi-disciplinary team to sort out social problems encountered by residents.
  • Maintain records of beneficiaries and manage the Electronic Case Management System (eCMS), and report in EPES on ICP updates.
  • In-charge of publicity outreach to relevant Agencies, Hospitals etc. for purpose of increasing the muster for the Residents’ Care and TIL programmes.
  • Explore job opportunities for the beneficiaries.
  • Compile and provide data to MSF and any other relevant agencies.
Requirements
  • Registered Social Worker with Bachelor’s or Graduate Diploma in Social Work
  • More than 5 years of experience in Social Service Agencies, MOH or NH in similar capacity.
  • Experience in leading a small social work team.
