Palliative Home Care Senior/ Staff Nurse (Criteria : ICU/ Palliative Trained) - Up 6.5K #HJC

Overview

Job type: Senior/Staff Nurse in Palliative Home Care. Schedule: Monday to Friday, 8.30am to 6pm. One day per week may involve covering 3-5 homes depending on patient loads. Patient Profile: Palliative patients.

Responsibilities

• This role will be abit more like case management – manage the care plan for the patients.
• Conduct home visits to provide palliative care and recommend psycho-social support to terminally ill patients and their families.
• Collaborate with doctors, nurses and medical social workers to provide all-in-one professional care to patients at home, at all hours, seven days a week.

Requirements

• ICU/Palliative Trained qualifications or criteria.
• Strong communication and patient-family support skills relevant to palliative care.
