Job description

The role leads the planning, implementation and optimisation of hospital-care-led community mental health programmes such as ASCAT and Aftercare, for one healthcare cluster. Reporting to the Assistant Director, the role oversees service delivery, system linkages, programme performance and stakeholder partnerships within the cluster, ensuring alignment with the Community Mental Health (CMH) Masterplan and achieving measurable outcomes for clients and caregivers in the community.

  • Cluster Programme Leadership: Lead and oversee the implementation of hospital-care-led community mental health programmes (e.g. ASCAT, Aftercare) for the assigned healthcare cluster, ensuring alignment with the CMH Masterplan.
  • Service Delivery & System Integration: Identify and address system gaps, workflow issues and process challenges to improve mental health service coordination and client care continuity.
  • Stakeholder & Network Management: Build and manage strong working partnerships with hospitals, community partners and other stakeholders to strengthen the regional mental health ecosystem.
  • Programme Monitoring & Performance Management: Track, analyse and report on programme KPIs and outcome measures; contribute data and insights for cluster‑level service planning and continuous improvement.
  • Quality & Care Model Enhancement: Support the development and implementation of quality frameworks and service standards to ensure programme effectiveness and consistency across providers.
  • Capability Building Support: Work with providers and stakeholders to identify learning gaps and support the roll‑out of mental health capability building initiatives across the cluster.
  • Funding & Resource Coordination: Support budget planning, resource allocation and funding utilisation for cluster programmes, ensuring accountability and efficient use of resources.
  • Policy & Strategy Input: Contribute cluster‑level insights, ground feedback and implementation learnings to inform broader policy, service development and regional mental health strategies led by the Assistant Director and Head of Department.
Job Requirements
  • Bachelor’s Degree in Social Work, Psychology, Counselling or related field; postgraduate qualification would be an advantage.
  • Minimum 8 years of relevant experience, with 2–3 years in a supervisory/management role.
  • Experience in community mental health, healthcare or social service systems, including multi‑agency or cross‑sector collaboration.
  • Strong background in programme implementation, monitoring and evaluation.
  • Proven stakeholder management, negotiation and facilitation skills across diverse organisations.
  • Strong analytical thinking, systems thinking and problem‑solving abilities.
  • Experience in service development, design thinking or organisational development preferred.
  • Resilient, adaptable, able to manage ambiguity and complexity in a healthcare ecosystem.
