Central to Mount Alvernia Hospital’s mission is a commitment to giving back to the community and serving the vulnerable. This legacy is being carried forward by the Community Outreach Team. Join our burgeoning team to drive the growth in our hospital-based programme providing highly subsidised tertiary care. In this role, you will:
Spearhead the expansion of our existing hospital-based programme to serve low-income transnational families in Singapore.
Build channels of referrals from social service agencies in identified new target segments of persons with disabilities and mental health for vulnerable youths.
Formulate programme policies, organise operating procedures and develop strategies to meet long-term goals.
Build and nurture strong relationship with key stakeholders including social workers, volunteer doctors and hospital management on outreach programmes.
Develop a cloud-based case management and data repository for the tracking of cases and analysis of statistics for KPI reporting.
Collaborate closely with clinic-based and offsite Outreach colleagues to bring about a holistic outreach healthcare offering for the community.
Bachelor’s degree in any discipline.
At least 5 years of experience in social services especially in programme management.
Healthcare experience advantageous.
Strong project management skills with demonstrated ability to drive initiatives from conceptualisation to implementation.
Detail oriented, possess strong reasoning with the ability to manage multiple projects and meet tight deadlines.
Excellent communication and presentation skills with the ability to build rapport with diverse stakeholders.
