A healthcare organization in Singapore is seeking a Case Manager (Social Worker) to join their COMIT Team. This role involves overseeing case management and ensuring effective program delivery, including triaging processes, psychoeducation, and caregiver support. The ideal candidate should possess a Bachelor's degree in Social Work and have at least 3 years of relevant experience, alongside strong communication skills. This position offers a collaborative environment to engage with community partners and stakeholders.
The Case Manager (Social Worker) will be part of the COMIT Team who will ensure service delivery is in accordance to the intended programme objectives and care model for the programme as outlined under the terms & conditions of the Letter of Award/Addendum.
The Case Manager will work closely with the Case Integration Lead to oversee the triaging process, reviews and work towards achieving the COMIT goals of timely assessment, provision of intervention and support eg. psychoeducation, case management and counselling, as well as caregiver support and training.
In addition, the Case Manager (Social Worker) may also be involved in networking with other community partners or agencies, for purpose of collaboration and to facilitate co-management of cases where applicable and other adhoc duties assigned.
This candidate will be one of the key members driving the COMIT project.
