Responsibilities
  • Work with the identified patient to plan and coordinate his/her care, activities and discharge based on his/her clinical, physical, psychosocial, cultural and other identified needs.
  • Function as a resource person in the assessment of complex patient care needs / situations, plan for interventions and for adolescent patients, provide appropriate interventions such as family therapy
  • Monitor discharge care plans, and evaluate the effectiveness of care at the next level in continuum
  • Collaborate proactively with hospital’s multi-disciplinary team members, provide coordination and continuity of patient care across the healthcare continuum with primary care and community partners to facilitate smooth transition of patient to the home and into the community.
  • Cooperate with the community service partners to follow up with care plan if necessary, when there is variance from the discharge care plan.
  • Assist to plan, coordinate and conduct mental health related educational activities for NUHS staff, primary care partners, community mental health partners and the public.
  • Conduct house visits to patient and interact with patient’s caregiver/family members when needed.
  • Monitor and maintain programme database and indicators
Requirements
  • Minimum Bachelor Degree in Social Work, Counselling or clinical training in mental health
  • At least 2 years’ experience in case management or related field.
  • Good understanding of healthcare and social support system
  • Able to facilitate professional relationship between patient, family, members of multi-disciplinary team and community service providers
  • Excellent interpersonal skills and communications skills with fluency in English and at least one other language or local dialect.
  • Contractual till 31 Mar 2028
