St Joseph's Home, a Person‑centred residential healthcare facility located in Jurong West, is redefining the way we take care of our elderly, and would like to invite suitable candidates to apply for the position of:

Care Coordinator

The Care Coordinator plays a pivotal role in supporting the nursing and social work teams in delivering holistic psycho‑social care to residents. This includes enhancing residents' quality of life, facilitating family involvement through Advance Care Planning (ACP), rebuilding family bonds, and providing caregiver and bereavement support where required.

  • Provides pre‑admission and admission supports which include assessing and understand residents’ cognitive and functional abilities with input from nursing, physiotherapy, and occupational therapy teams
  • Assist Social Workers (SWs) in conducting non‑complex financial assessments
  • Build rapport with residents and their next‑of‑kins to understand their psycho‑social needs
  • Collaborate with multidisciplinary teams to gather holistic information and support care planning
  • Participate in interdisciplinary care planning and monitor intervention progress
  • Ensure objective evaluation and documentation of psycho‑social interventions
  • Provide timely updates during multidisciplinary meetings and support SWs in preparing social reports with appropriate consent
  • Identify individual resident interests to develop meaningful and stimulating activities
  • Coordinate with volunteers to implement activities and evaluate their impact on resident quality of life
  • Support the Befriender Program in collaboration with the Pastoral Care Team
  • Support in Advance Care Planning (ACP) and Life Reviews
Qualifications
  • A Diploma in Social Work/Community Service or degree in any other related disciplines.
  • At least 2 years of relevant work experience.
  • Proficient in MS Office programs, especially in Word and Excel programs.

  • A good team player with the ability to work independently with minimal supervision
  • Fluent in spoken and written English; the ability to converse in Mandarin or dialects is an asset for communicating with dialect‑speaking clients
  • Possesses a positive attitude toward learning

  • Able to multitask effectively
