Brief Summary:

Join a dedicated team where you will oversee patient care services and enhance nursing practices in a supportive environment. Lead the way in delivering compassionate home-based health and palliative care.

Responsibilities:

1. Provide direct patient care and support nurses in managing complex care needs.
2. Supervise and guide nursing staff during home visits, ensuring compliance with documentation in clinical systems.
3. Participate in interdisciplinary team meetings to coordinate patient care and address clinical needs.
4. Collaborate with nursing leadership to develop and implement training programs for nursing staff.
5. Monitor and evaluate nursing practices, identifying areas for improvement and implementing necessary changes.
6. Support and implement initiatives that promote the well‑being of nursing personnel.
7. Participate in the recruitment and development of nursing staff to ensure operational efficiency.
8. Work with clinical and administrative leads to guarantee compliance with home care and hospice service standards.
9. Develop and coordinate internal audits to uphold nursing standards and lead quality improvement initiatives.
10. Perform any additional tasks related to organizational requirements.

Requirements:

1. Registered Nurse with a valid practicing certificate from the Singapore Nursing Board.
2. Bachelor's Degree in Nursing is mandatory.
3. Advanced/Specialist Diploma in Gerontology, Palliative Care, Community Nursing, or related fields is preferred.
4. Minimum of 8‑10 years of clinical experience, with at least 3‑5 years in community or hospice care settings.
5. Proven experience in supervising nursing staff for a minimum of 3‑5 years.
6. Strong verbal and written communication skills, along with the ability to engage with management and external partners.
7. Demonstrated leadership abilities with a track record of teamwork.
8. Familiarity with various software applications (e.g., MS PowerPoint, MS Excel, MS Word) to enhance training and teaching efforts.
