Areyou an enthusiastic, motivated Dietitian with a keen interest in leading theChildrens and Young Peoples Diabetes Dietetic Service? Do you have apassion for supporting children and their families and carers, workingcollaboratively with the multidisciplinary team? We are looking for anexperienced and enthusiastic Dietitian to join our Childrens Dietetic Teamto take up this exciting post, working 30 hours per week (0.8WTE). You willbe joining a compassionate, award winning and innovative diabetes team. Youwill work with two Band 6 and one Band 5 diabetes dietitians.

TheLeeds Children and Young Peoples Diabetes Service provides medical, nursing,dietetic and psychological support to children and young people (0-19 yearsof age) living with diabetes.

Asone of the largest childrens diabetes services in the UK, our Diabetesspecialist team takes care of over 500 young patients, from diagnosis throughlong-term follow-up to achieve the best long-term health outcomes.

Wesupport our patients, their families and school staff in achieving optimalblood glucose management through blood glucose monitoring, carbohydratecounting, insulin dose adjustment and management of exercise, illness andhypoglycaemia (low blood glucose). We use the latest in diabetes relatedtechnology to help our patients and families in living with their diabetes.

Main duties of the job

Provide advanced, highly specialised dietetic care to children and young people and their families and carers at a local and regional level as the lead diabetes dietitian. This will include our in-patient and out-patient services and our programme of education. Provide and lead aspects of clinical training for dietetic learners and other staff. Support the Childrens Clinical Dietetic Manager with the day-to-day running of the team, with leadership and line management roles.Lead and co-ordinate dietetic research, audit and guidelines within Children's Diabetes Team alongside the MDT. Support and collaborate around advances in technology and digital solutions.

Job responsibilities

Provideadvanced, highly specialised dietetic care to patients at local and regionallevel. This will include advice to patients and all healthcare staff. Provideand lead aspects of clinical training of dietetic students. Support Clinicaldietetic Manager in co-ordinating activities of the team and providingsupport and supervision to staff. Lead in research and audit withinspecialised clinical area and disseminate findings.

3. JOB DIMENSIONS

Thepost holder will:

3.1 Lead in a clinical specialist area.

3.2 Manage own caseload and supervise thecaseloads of the other dietetic staff.

3.3 Assist and support the ClinicalDietetic Manager/line manager in the day to day management of the team ofdietitians and students.

3.4 Deliver and supervise training tohealthcare staff, dietitians and students at local and regional level, actingas a resource in the specialist area.

3.5 Be a core member of themultidisciplinary care team, liaising with other members of the team toprovide integrated dietetic care.

3.6 Initiate and co-ordinate research andaudit projects within specialised area and disseminate findings at conferencelevel.

3.7 Be an active member of the out of hoursservice.

5. KNOWLEDGE, SKILLS AND EXPERIENCEREQUIRED

5.2 HCPC Registration as a Dietitian.

5.3 Experience of working in a seniordietetic role.

5.4Willing to undergo any training asrequired to enable performance in thejob.

5.5 Advanced knowledge of current issues inclinical specialist area.

5.6 Able to work within a team,demonstrating leadership, initiative and confidence.

9. CORE KNOWLEDGE AND UNDERSTANDING

9.1 Ability to perform advanced nutritionalassessments in respective clinical area, interpret and plan relevant dieteticcare programmes for patients with highly complex nutritional and medicalneeds which requires judgement and consideration of a range of treatmentoptions.

9.2 Advanced knowledge of biochemistry,physiology and specialised pharmacology.

9.3 Understanding of the calculationsrequired in dietetic assessment.

9.4 Ability to correctly interpret complexnutritional impairments, biochemical parameters and fluid balance. This willsinvolve nutritional diagnosis including interpretation of biochemistry,anthropometrics, clinical condition, social factors and diet histories inpatients with highly specialised and complex nutritional needs.

9.5 Highly specialist knowledge ininterpreting complex clinical information to develop and monitor nutritionalcare plans in a specialised dietetic area. This will include the prescriptionof dietary treatment, prescribable nutritional products, enteral and totalparenteral nutrition (TPN).

9.6 Responsible for safe dischargeplanning, including co-ordination of discharge of patients on home enteralfeeding.

9.7 Able to evaluate, develop and implementpractice using evidence based practice and critical appraisal.

9.8 Ability to advise and suggestnutrition-related medications.

9.9 Knowledge of clinical governance,quality of care and evidence based practice in specialist area.

9.10Awareness of national guidance anddriving factors in specialist area.

10. PRINCIPAL DUTIES & AREAS OFRESPONSIBILITY

10.1 Clinical and professional

10.1.1 Be an autonomous practitioner, clinically,legally and professionally responsible for own caseload.

10.1.2 Responsible for undertaking dieteticclinical duties in specialised clinical area, managing and prioritising ownclinical caseload.

10.1.3Collect analyse and record relevant patient information from clinical andpatient sources to develop effective nutritional care plans.

10.1.4 Communicate complex dietetic treatment andadvice of patients to healthcare staff, professionals and patient carers.This will involve co-ordinating nutritional care with other Trusts andregional centres.

10.1.5 Undertake advanced assessments of patientswith diverse and highly complex nutritional issues where investigative andanalytical skills are required to formulate accurate treatment plans.

10.1.6 Responsible for providing highlyspecialised dietary advice within an in-patient and out-patient setting, topatients in a distinct specialist area. Eg advice to renal dialysis patienton calcium and phosphate management, oncology patients on enteral feeding,cystic fibrosis patient on fat intake and enzyme replacement therapy.

10.1.7 Responsible for developing and maintainingown expertise and agreed level of expertise of team members by updating ownspecialised clinical knowledge and skills. This will include developingregional networks within the specialist and attending local, regional andnational professional interest group meetings.

10.1.8 Participate in the appraisal process as anappraiser and appraise, developing a strategy for personal and continuousprofessional interest group meetings.

10.1.9 Responsible for implementing clinicalgovernance and risk assessment in respective specialised clinical area aspart of continuing quality improvement e.g. advise patients and staff oninfection control issues around enteral feeding.

10.1.10Undertake research and audit within specialist area using researchmethodology. This wills involve identifying, co-ordinating and evaluatingresearch and audit projects to measure clinical effectiveness and promoteevidence based practice.

10.1.11Expectation to attend regional and national conferences as a delegate andpresenter. This will include both verbal and poster presentations.

10.1.12Undertake any other duties appropriate to the grade as requested by linemanager.

10.2 Education and training

10.2.1 Devise and implementspecialised nutritional care plans for inpatients and outpatients byinterpreting national guidelines, standards and National Service Frameworksfor use in specialist area. This will involve advising patients, relatives orcarers, individually or in a group setting.

10.2.2Provideeffective oral and written specialised dietetic advice and information topatients, and their carers, to enable achievement of dietary aims.

10.2.3 Lead the development of resources used inthe education and training of patients, health professionals and the publicat local and regional level.

10.2.4Provide postgraduate training on a range of subjects within specialist areato multidisciplinary groups and audiences at local and regional level. Thiswill involve medical, dietetic, nursing staff and other healthcareprofessionals.
