Primary Mental Health Clinical Team Lead

An exciting opportunity has arisen for a Band 7 Primary Care Mental Health Clinical Team Lead to join South Kerrier Primary Care Network, working directly within GP surgeries to strengthen the delivery of proactive and preventative mental health care. Embedded in general practice, the postholder will work closely with GPs and the wider multidisciplinary primary care team to support the early identification and management of complex mental health presentations, providing specialist input within the GP setting and ensuring appropriate escalation to secondary care when required. The role sits within a small PCN-based mental health team alongside a Band 6 Mental Health Practitioner and a Health and Wellbeing Coach, working closely with Social Prescribing colleagues, GP practices, secondary care services and voluntary and community sector partners to support integrated, person-centred care, while also providing clinical supervision and contributing to the development and delivery of group-based and one-to-one interventions for primary care patients.

Main duties of the job

The role of Primary Care Mental Health Practitioner is fundamental to the developing of these innovative new teams and mental health services based within networks of GP Practices (PCN's). To provide a sustainable, person‑centred system of community mental health care, which delivers better access to a broader range of care options, reduced referral to specialist services, improved patient experience and outcomes. It will aim to include evidence‑informed approaches that are known to impact on both mental and physical health.

The role will support the ongoing development and mobilisation of integrated primary care mental health services. The role provides and promotes early assessment, treatment and/or onward bridging to community resources and support and improved access to evidence‑based interventions for people with serious mental illness. The role will help people to focus on achievable goals and access local community resources. The service will be for people over 18 years (including carers) with a specific focus on:

  • People whose mental health needs that do not meet the access criteria for IAPT or secondary care.
  • People with serious mental illness who are in recovery and stable in secondary care mental health services who could be cared for within Primary Care

Qualifications
  • Relevant professional qualification e.g. Dip OT, Dip SW, RMN, Clinical Psychology Grade A, or equivalent
  • Non medical prescriber
  • Further qualification in mental health work
Experience
  • Experience of working in a mental health care setting and of liaising and working with GPs
  • Suitably trained and experienced in the delivery of short term, focussed evidence based interventions
  • Experience and an awareness of common acute and long‑term conditions that are likely to be seen in general practice
  • Experience of developing services/project work
