BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time role providing home health visits for our senior living office in Blue Bell, PA. This office services adult and geriatric clients across the Mainline area.
As a home care Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities include:
- Making home visits to clients in designated areas.
- Performing diagnostic tests and measurements, including mobility, joint range, stability, gait, strength, endurance, and safety assessments.
- Implementing wound care interventions with evidence-based dressings and topical agents.
- Developing and updating individualized care plans, including manual exercises, gait training, and balance interventions.
- Assessing and revising care plans, collaborating with nursing staff as needed.
- Educating clients and families on rehabilitative activities to promote health, safety, and independence.
- Documenting observations and interventions using electronic tablets.
Qualifications:
- Current PA Physical Therapist (PT) license.
- Graduation from an accredited Physical Therapy program.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Proficiency with computers; experience with EMR preferred.
