Qualifications

• Experience in carrying out initial risk and needs assessments in service users' homes.
• Working in partnership with service users and healthcare professionals.
• Successfully passed DBS screening.

Responsibilities

• Plan effective care worker rotas with Care Coordinators.
• Carry out regular reviews of individual services.
• Participate in multi-disciplinary reviews as required.
• Shadow new care workers to ensure their competence.
• Observe care worker practices as part of supervision.
