Job description
Overview

Community Staff Nurse, Band 5 – Bexley Twilight District Nursing Services. Permanent: Part Time – From 15 to 25 hours weekly. The Twilight Service operates between 17:00 hrs – 22:00 hrs or between 18:00 hrs – 23:00 hours. The service is provided by Oxleas NHS Foundation Trust.

Responsibilities
  • Deliver a high standard of nursing care to patients in their own homes, including monitoring of blood glucose, administration of insulin, cannulation, administration of IV antibiotics via infusion, wound care, catheterisation and end of life care.
  • Carry out full and thorough clinical and risk assessments, holistic assessments, and escalate risk to senior staff where necessary.
  • Develop and implement individualised care plans to deliver high quality, evaluated care; monitor and document care following trust standards.
  • Carry out physical health assessments and regular monitoring, ensuring nutrition, hydration, comfort, hygiene and documentation meet quality standards.
  • Demonstrate professionalism by challenging and reporting poor practice when observed, while maintaining privacy and dignity of patients.
  • Engage in regular clinical supervision and reflective practice; support junior staff and contribute to annual personal development plans with line management.
  • Be committed to evidence-based best practice and act as a role model for junior staff, understanding personal values and their impact on care relationships.
  • Be prepared to travel across the Borough in a timely manner; a full driver’s licence and access to a car for work purposes are essential, alongside a business insurance certificate.
