Job description
Overview

We’re looking for a passionate and proactive Transformation Manager to deliver high-quality care to our patients. This is a fantastic opportunity to contribute to a forward-thinking healthcare organisation while working in a supportive and inclusive environment.

Location: Hybrid - Remote with regular travel to site

Hours: 40 per week

Contract Type: Permanent

Key Responsibilities
  • Support the planning, design, and delivery of complex transformation projects across services.
  • Develop project plans, business cases, and benefits realisation plans.
  • Manage project governance and risk management in line with agreed budgets.
  • Build strong relationships with internal and external stakeholders including clinicians, managers, commissioners, and service users across Evergreen Life and Omnes Healthcare.
  • Facilitate co-design workshops, engagement sessions, and regular communications to ensure inclusive and informed change processes.
  • Apply recognised change management methodologies (e.g. Lean, Six Sigma) to support operational teams and embed sustainable improvements.
  • Support teams in adapting to new models of care, digital systems, and new ways of working; utilise influencing and persuasion skills to establish the basis for successful delivery of service development and transformation workstreams.
  • Use data to identify opportunities for improvement and track progress against KPIs, ensuring timely communication to the leadership team.
  • Support the delivery of cost improvement, service redesign, and digital transformation initiatives; monitor performance and ensure compliance, feeding learnings into subsequent contracts.
  • Promote a culture of continuous improvement and innovation; coach and mentor teams to build change capability across the organisation.

Role Requirements
  • Educated to degree, master’s level or equivalent diploma level or equivalent experience.
  • Proven experience of leading complex transformation programmes in healthcare or a similar regulated sector.
  • Strong project and programme management skills (e.g. PRINCE2, MSP).
  • Experience of using quality improvement and change management tools.
  • Excellent communication, negotiation, and facilitation skills; influencing and conflict resolution abilities.
  • Ability to work across organisational boundaries and influence at all levels, developing consensus and ensuring support for delivery of projects/programmes of work.
  • Analytical, with ability to interpret and distil complex data to inform improvement; experience with digital transformation.
  • Experience using software to support change (e.g. Word, Excel, PowerPoint, MS Project, Visio).
  • Resilient and able to balance competing priorities and deadlines.
