Overview

We’re looking for a passionate and proactive Transformation Manager to deliver high-quality care to our patients. This is a fantastic opportunity to contribute to a forward-thinking healthcare organisation while working in a supportive and inclusive environment.

The Transformation Manager delivers strategic change programmes to improve the quality, efficiency, and sustainability of services across the organisation. Working collaboratively with clinical, operational, and corporate teams, the postholder ensures that change initiatives align with organisational priorities and deliver measurable benefits for patients, staff, partners and stakeholders. The post holder may work flexibly across primary care and secondary care teams as required, based on local priorities.

Main Duties
  • Support the planning, design, and delivery of complex transformation projects across services.
  • Develop project plans, business cases, and benefits realisation plans.
  • Manage project governance and risk management in line with agreed budgets.
Stakeholder Engagement
  • Build strong relationships with internal and external stakeholders including clinicians, managers, commissioners, and service users across Evergreen Life and Omnes Healthcare.
  • Facilitate co-design workshops, engagement sessions, and regular communications to ensure inclusive and informed change processes.
Change Management
  • Apply recognised change management methodologies (e.g. Lean, Six Sigma) to support operational teams and embed sustainable improvements.
  • Support teams in adapting to new models of care, digital systems, and new ways of working.
  • Utilise influencing and persuasion skills to establish the basis for the successful delivery of service development and transformation workstreams.
Performance and Improvement

Use data to identify opportunities for improvement and track progress against KPIs, ensuring accurate and timely communication to the leadership team.

  • Support the delivery of cost improvement, service redesign, and digital transformation initiatives.
  • Monitor performance of service development and transformational workstreams, ensuring compliance and identifying areas for further development, feeding these into subsequent contracts.
Leadership and Culture
  • Promote a culture of continuous improvement and innovation.
  • Coach and mentor teams to build change capability across the organisation.
Role Requirements
  • Educated to degree, master’s level or equivalent diploma level or equivalent experience.
  • Proven experience of leading complex transformation programmes in healthcare or a similar regulated sector.
  • Strong project and programme management skills (e.g. PRINCE2, MSP).
  • Experience of using quality improvement and change management tools.
  • Excellent communication, negotiation, and facilitation skills.
  • Influencing and conflict resolution skills.
  • Ability to work across organisational boundaries and influence at all levels, developing consensus and ensuring support for the delivery of projects/programmes of work.
  • Analytical, proactive in identifying obstacles and thinking ahead; able to use analytical techniques to identify several solutions.
  • Ability to interpret and distil complex data, both quantitative and qualitative, to inform improvement.
  • Experience with digital transformation.
  • Experience of using software packages to support change (e.g. Word, Excel, PowerPoint, MS Project, Visio).
  • Resilient and able to balance competing priorities and deadlines.
