Overview

As a Team Leader at Avery Healthcare, you will be responsible for supporting the home management team in delivering high-quality, person-centered care. Your focus will be on creating a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. Your role includes leading in the care planning process, assisting with resident discharges, monitoring resident well-being, and ensuring safe systems of work.

Job responsibilities
  • Package Description: Shift Details - Night shift with alternate weekends.
  • Lead in the person-centred care planning process by implementing, monitoring, undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assist in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents\' next of kin fully informed, where appropriate.
  • Lead with monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
  • Practice safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
