Main duties of the job

As a Team Leader at Avery Healthcare, you will support the home management team in delivering high-quality, person-centered care. You will implement, monitor, and evaluate care plans, assist with smooth transitions of resident discharges, and monitor resident well-being. ABOUT THE ROLE

Your focus as a Team Leader will be to support the home management team in delivering high-quality care by providing a safe, caring, innovative, open, supportive and inclusive environment for residents, visitors, and staff. Other responsibilities will include:

  • Leading the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assisting in the smooth transition of discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed where appropriate.
  • Leading the monitoring of resident well-being, noting changes impacting care and care planning, and ensuring that changing needs are met and written records are amended accordingly.
  • Practicing safe systems of work across tasks and moving and handling of residents, assessing risks and ensuring personal safety and safety of residents, visitors, and staff.
