As a Team Leader at Avery Healthcare, you will support the home management team in delivering high-quality, person-centered care. You will implement, monitor, and evaluate care plans, assist with smooth transitions of resident discharges, and monitor resident well-being. You will practice safe systems of work and demonstrate the ability to lead staff effectively.
