The Community Mental Health Team are excited toannounce the position of a Team Clinical Lead within our PCN teams. The successful candidate will be able toprovide advice, guidance and clinical supervision to members of the CommunityMental Health Team (CMHT) along with working towards individual formulationsfor our service users. They will alsosupport the Senior Operational Managers with business and service delivery,whilst continuing to develop the functions and structure of the organisation tobest meet the mental health needs of some of our most vulnerable clients aswell as holding a small caseload.

Main duties of the job

You will:

Offer clinical leadership and maintain, monitor andfacilitate the delivery of a high-quality Mental Health Service

Provide strong professional/managerial and clinicalleadership to the multi-disciplinary team

Provide strong leadership in respect of developingexcellence in practice

Ensure National Standards and local guidelines, policies andcare pathways relating to care and treatment are effectively delivered

Promote and ensure service users and carers are fullyinvolved in the services they receive

Work across N E Lincolnshire providing mental health adviceand leadership based on an agreed operational policy

Assist in developing and evaluation of models of care toenable effective communication

Participate in multi-disciplinary team meetings

Job responsibilities

Thepost holder will be responsible for the development and implementation of theday-to-day Clinical leadership of the Team which will form part of one of thefour Primary Care Networks within North East Lincolnshire.

Theywill offer clinical leadership, ensuring the maintenance and promotion of highstandards of person centred care for service users as well as assisting withthe development of staff within the team.

Theywill be expected to maintain, monitor and facilitate the delivery of a highquality Mental Health Service in the most efficient and effective manner withinexisting constraints.

Toprovide strong professional/managerial and clinical leadership to theMulti-disciplinary Team.

Toprovide strong leadership in respect of developing excellence in practice,ensuring that robust practice and personal development initiatives areavailable.

Toensure national standards and local guidelines, policies and care pathwaysrelating to care and treatment are effectively delivered for the promotion anddelivery of quality services.

Topromote and ensure service users and carers are fully involved in the servicesthey receive.

Tosupport good quality care, by ensuring the highest standard of environment isprovided, in line with National Guidance and local service specification.

Tosupport and provide co-ordinated and integrated care, by ensuring effectiveworking relationships exist between the services.

Totake the lead role in the promotion and implementation of person centred care.

Toensure the care offered is at the level each one of us would want our ownfamily to receive.

Tosupport the staff through a change programme in line with Navigo work streamsin line with the CMHT Transformation model.

MAIN DUTIES / RESPONSIBILITIES:

CLINICAL:

Towork across North East Lincolnshire providing mental health advice andleadership based on an agreed operational policy and evidence based practice.

Toassist in developing and evaluation of models of care to enable effectivecommunication and to facilitate best practice taking into account availableresources.

Participatein multi-disciplinary team meetings, meetings associated with the MentalCapacity Act/Mental Health Act/Continuing Healthcare and team meetings.

Tomonitor, evaluate and ensure maintenance of the highest achievable standards ofcare.

Coordinatetransfer or referral of service users to other teams or services whererequired.

Prioritise,plan and coordinate own workload and activities in line with the changing needsof the team and service. This includes planning and monitoring clientmedication needs, making appointments for clients, conducting assessments,managing emergencies and enquiries, responding to crisis work within individualcaseload, ensuring patient records are up to date and supporting colleagues ona daily basis.

Havethe ability to work independently and to be able to resolve complexprofessional problems.

Crisisresponse to other staffs service users in the event of key workers absence,and delegation from manager.

Toparticipate in the implementation of an effective clinical supervisionstructure across the service

Assessskill mix and identify areas of development within the Team.

Participatein the continued development of a comprehensive Risk Assessment and Managementstrategy for the service.

Provideprofessional support to staff by devising and implementing a framework forcritical incident analysis and de-brief.

Toprovide and set up appropriate clinical training for staff across the Team.

Totake managerial responsibility for the Service in the absence of the SeniorOperational Manager.

Tobe a positive team member, act as a role model for all staff grades and toquestion concerns in a direct and responsible manner.

Toensure that all staff participate in a programme of PDR in line with NAViGOpolicy.

Toensure that the actions and interventions of all staff are in accordance withrelevant professional codes of conduct, the Mental Health Act (1983), theMental Health Act Code of Practice and the Mental Capacity Act (2005)

Towork with and support the Senior Operational Manager with sound financialapproaches to finances and budgets.

Toensure that appropriate systems are in place in support of all aspects of dayto day administration and forward planning of human resources in an effectiveway.

Toassist in the development of, and to ensure the implementation and monitoringof local policies on safety.

Tosupport providing a safe environment for service users, staff and visitorsthrough effective Health and Safety at Work awareness and provision of goodstandard relating to the ward environment.

Liaisewith other members of the MDT and act as a link between the acute areas andCommunity.

Participatein meetings as appropriate and act as chair for relevant meetings.

Qualifications
  • A Professionally Registered Nurse/Occupational Therapist/Social Worker
  • IT Skills
  • 1 year experience of leadership at Band 6
  • Portfolio of relevant post qualifying education
  • Degree or equivalent
  • Non-medical prescribing qualification or willingness to work toward achievement of, advanced practitioner or responsible clinician status or willingness to work toward achievement of, including undertaking of relevant diagnostic practitioner training.
  • Post graduate qualifications in any aspect of Mental health or community care
  • Therapy qualification or experience in skill acquisition for complex mental health conditions/disorders.
  • Teaching experience/qualification
