Care Assistant / Support Worker - Joint Community Rehabilitation (JCR)

Location: Lewes, Haven, High Weald, Eastbourne, Hailsham, Seaford

Pay: £14.13 per hour

Contract: Permanent - Pro-rata | Flexible shifts | 4-week rota

Hours: 12-36 hours per week | Every other weekend | Shifts: 7am-12pm, 1pm-5pm, 6pm-10pm

Driver with own car required

About the Role

Join our Joint Community Rehabilitation (JCR) team and make a real difference! JCR provides up to 6 weeks of support to adults in their own homes after illness or injury, helping them regain independence and confidence. We operate 365 days a year between 7am and 10pm, offering flexible working patterns.

As a Support Worker, you'll deliver reablement support, working towards incremental goals using motivational techniques. You'll help clients with daily living tasks, medication, and prescribed exercise programmes, enabling them to live their best life.

What You'll Do
  • Provide practical and emotional support to clients in their homes.
  • Assist with personal care, medication administration, and health-related tasks.
  • Use excellent communication skills to understand and meet client needs.
  • Record and monitor information accurately, reporting concerns promptly.
  • Work collaboratively with families, carers, and professionals.
