Overview

Interview date: To be published shortly. This advert covers three specialties—Respiratory Medicine, Care of the Elderly and Renal Medicine. The post holder will be a member of the medical team and will perform specialist duties in areas such as Acute Medicine, Renal Medicine and Respiratory Medicine, working with consultant teams. The post holder will be pivotal in the clinical management of presentations to Jersey General Hospital. The post holder will work across all domains of Medicine including Medical Wards, Specialist Teams and Outpatients, and Ambulatory SDAC areas where appropriate.

These specialty posts commit to the assessment and ongoing care of medical patients in Jersey General Hospital.

Main duties of the job
  • To undertake clinical practice within the medical wards ensuring that personalised, evidence informed medical care plans are delivered to all patients and that they are appropriately documented and regularly evaluated.
  • To work in close partnership and liaison with Primary Care to provide telephone advice including provision of hospital avoidance initiatives.
  • To undertake inpatient management of medical patients including participation on ward rounds and board rounds.
  • To provide clinical advice & support to primary care colleagues and other consultants within Health & Care Jersey medical model.
  • To assist, where appropriate, with the management of patients of a higher acuity within the Enhanced Care Area.
  • To provide advice and timely medical reviews and interventions as part of the interdisciplinary model of care.
  • To engage with wider members of the MDT.
  • To liaise with tertiary referral centres as appropriate to provide patient centred care & service improvements as directed by consultant plans.
  • Maintain good quality up to date medical records including timely discharge letters and summaries.
  • To ensure appropriate documentation and coding is used to provide consistent audit and monitoring of the service.
  • To adhere to local prescribing guidelines.

Qualifications
