Job description

We are looking for an enthusiastic, well‑rounded Physiotherapist or Occupational Therapist to join the Manchester Community Response team as a Band 6 practitioner supporting early discharge from hospital.

Responsibilities
  • Provide timely home assessments for patients following discharge from hospital.
  • Develop comprehensive treatment plans supporting goals from other therapy, nursing and social care professionals.
  • Deliver specialist assessments and interventions, including equipment provision, home exercise programmes, and cognitive assessments.
  • Coordinate with nursing and social services to ensure smooth transition home.
  • Work flexible hours to support a 7‑day weekend and bank holiday service enabling additional discharges.
  • Maintain an active CPD portfolio and engage with training opportunities within the team.
  • Collaborate with the Manchester Community Response team when service needs arise.
Qualifications – Essential
  • BSc in Physiotherapy or Occupational Therapy.
  • HCPC Registration.
  • Evidence of relevant post‑registration training and CPD.
  • Experience of timely assessments and care‑plan delivery in a Band 5 rotational role.
  • Knowledge of complex health, social and psychological needs of older adults and evidence‑based practice.
  • Strong clinical reasoning, organisational and prioritisation skills.
Qualifications – Desirable
  • Member of a professional body.
  • Relevant postgraduate degree.
  • Previous Band 6 experience or community service experience.
  • Involvement in audit, research or service development.
  • Experience of clinical supervision and student mentoring.

  • Band 6 – £38,682 to £46,580 a year, pro‑rata.
