A local health service provider in Merthyr Tydfil seeks an experienced Occupational Therapist to join their acute medical team. This permanent full-time position involves providing specialized assessments and interventions to patients, ensuring effective discharge planning, and collaborating with multidisciplinary teams. Candidates should hold a relevant degree and be HCPC registered, with strong communication and organizational skills. The role offers a competitive salary range of £39,263 to £47,280 per annum.

As part of a close MDT, you will work together with your patients to identify meaningful goals, provide assessments, identify appropriate interventions and support their independence and discharge from hospital.
The post holder will carry an independent caseload of patients and deliver occupational therapy intervention to a high standard with the ethos of providing evidence based, high quality interventions.
The successful candidate will work closely with the MDT, Principal OT, Discharge Liaison Team and Local Authorities to ensure effective cross boundary and agency working.

To provide specialist Occupational Therapy assessment, intervention, rehabilitation and discharge planning for patients to ensure a safe and timely discharge and to enable the person to remain in their home.
You will provide supervision and support to junior members of the team and student Occupational Therapists and have a passion for delivering evidence based, quality intervention using research, audit and quality improvement projects. The post holder will be flexible to service needs, possess excellent self-management skills and administer their caseload efficiently to ensure effective intervention focused on preventing avoidable admissions, rehabilitation and efficient discharge planning, depending on setting.
