Job responsibilities

The SPA Clinical Triage Nurse receives referrals from a diverse range of people & services, triages them and ensures they reach the right service promptly. The team also provides significant support to the Virtual Frailty Ward at daily MDT meetings. SPA Triage monitors the Health Call system for heart‑failure patients, ensuring out‑of‑range observations are escalated & acted upon. Palcall is a further element of the role, a telephone support & sign‑posting service for people within their last 12 months of life. Post holders need to be IT literate, able to multi‑task and require advanced communication skills.
