Job Summary

Main Duties

As a Social Worker Assistant you will carry out assessments with service users, collect and report information for social circumstances and social work assessments, manage personal budgeting and financial arrangements, maintain accurate records, and support service users on community leave. You will liaise with families, significant others, and external agencies to facilitate arrangements to meet individual outcomes, and promote adult safeguarding by referring issues to the Social Worker/TDT and Adult Safeguarding Team.

Responsibilities
  • Carry out assessments with service users, identifying the best and most appropriate outcomes.
  • Correspond and liaise with external agencies to facilitate and progress arrangements (e.g., housing, health).
  • Assist service users in understanding and managing their personal budgeting and financial arrangements.
  • Maintain and update timely and accurate records on relevant systems.
  • Aid and support the Social Worker role as required.
  • Support community meetings and accompany service users on community leave.
  • Gather information for Social Circumstances reports and social work assessments.
  • Complete and maintain visitor records and paperwork.
  • Cover meetings, telephone calls, and patient interviews when the Social Worker is not available.
