Job summary

A social care consultancy in Cardiff is seeking a responsible individual to arrange and monitor services as per care plans, advising families and liaising with agencies. Candidates should have experience working with children and families, as well as good communication and organisational skills. The role is mostly office-based with potential visits to service users.

Qualifications

• Experience of working with children and families through direct contact in a working environment.
• Ability to make relationships with service users and carers.
• Good communicative, organisational and administrative skills.

Responsibilities

• Arrange and monitor appropriate services in line with individual care plans.
• Provide advice and practical help.
• Complete annual care plan reviews and reassessments.

Responsibilities

• To arrange and monitor appropriate services in line with individual care plans.
• To provide advice and practical help.
• To liaise with voluntary and statutory agencies.
• To complete annual care plan reviews and or reassessments.
• To participate in team meetings and duty rotas.
• To undertake these duties in line with Council Policies and Procedures.
• To undertake training and self-development and to contribute to this process in others.

Requirements

• Ability to work independently and as part of a team.
• Ability to make relationships with se5rvice users and carers.
• Good communicative, organisational and administrative skills.
• Mostly office based but could be called on to visit service users as needed.
