Are you a compassionate and driven Senior Social Worker looking to make a real difference across Lancashire? Join our Short-Term Support Service, working in West Lancashire and nearby areas, where you'll help people live independently in a place they call home-through creative, person-centred support.
This is an exciting time to be part of Lancashire County Council. We've launched Living Better Lives in Lancashire, our new strengths-based vision for adult social care. We're embracing innovative practice and reshaping how we support our communities-and we need passionate social workers to help us bring this vision to life.
As part of our dynamic team, you'll be a key player in delivering short-term support that promotes independence and wellbeing. The team operates 7 days a week, providing a single point of access to community intermediate care services, including reablement and residential rehab. We also support hospital discharges and work closely with multi-disciplinary teams to prevent admissions and help people remain at home.
Using the Discharge to Assess model, we ensure assessments are carried out in the most appropriate setting-away from hospital environments. This role includes working 1 in 4 weekends, in line with service needs.

Social Work applicants must be registered with Social Work England (SWE) to be appointed as a Social Worker or have applied to register with SWE.
