Job description

An exciting opportunity has arisen for a Senior Practitioner to work within the Bolton North Specialist mental health team, previously named community mental health team. You will play a crucial role in the clinical leadership of the team and support staff to provide a high-quality service to service users and carers. We are looking for an experienced and enthusiastic practitioner who has a real passion for providing support to promote mental and physical health, wellbeing and recovery.

Responsibilities
  • Support the manager in all aspects of managing the services while providing clinical leadership and patient care within the team.
  • Support other practitioners in the development of their skills by offering highly developed risk assessments and management plans.
  • Lead with a positive and helpful attitude, be an excellent communicator, and motivate the team to deliver high quality and responsive care.
  • Participate in the service operational on call and operational on site rota as required (formerly known as bronze on call).
  • Act as a keyworker for a small number of highly complex cases in line with the Trust Care Programme Approach (CPA) guidance and policies.
  • Take responsibility for the day‑to‑day clinical leadership and development of a designated multidisciplinary team that is integrated, flexible, user‑focused and recovery‑oriented.
  • Deliver a safe, effective, and high‑quality service to service users with mental health needs, ensuring delivery to the relevant clinical pathways as outlined within the standard operational procedure.
  • Participate as a member of the Community Services Management Team in the overall management of community‑based secondary mental health services.
  • Promote the independence and well‑being of adults, assist social inclusion and valued community presence, and enable the widest possible expression of chosen lifestyle.
