A regional health services provider is seeking a CYP Starting Well SEND Practitioner in Northwich. This part-time role involves providing essential support to families with additional needs related to SEND. You will assess and coordinate support services, engage with families for timely interventions, and work collaboratively with health professionals. Applicants should have an NVQ level 3 in Health and Social Care or equivalent. A compassionate and inclusive approach is valued. Salary ranges from £27,485 to £30,162 pro-rata.

The SEND Practitioner will work collaboratively to support families with additional needs in relation to SEND. They will provide effective, needs-led family support for children with SEND or additional needs in relation to their health and development, ensuring children with emerging/complex needs receive the right support at the right time. Working under delegation from a Health Visitor, the SEND Practitioner will contact families and explore the outcome of any intervention. The role may include discreet pieces of work within family homes. This vacancy covers Northwich and Winsford District and involves 3 days in the office per week; a small amount of remote working is possible, but there is a requirement to be in the office on certain days.
