Job description
Overview

We welcome applications from both experienced staff nurses and newly qualified nurses who are keen to start a career in Public Health Nursing. You will join the service at an exciting time as we progress our 0-19 pathways, working more closely with Health Visiting services and our partners in education, social care and Early Help to take a family approach to the promotion of health and wellbeing, prevention and early intervention.

Post holders will work within a team under the direction of a Specialist Practitioner School Nurse to deliver the Healthy Child Programme 5-19 in schools and other education settings. You will develop health promotion activities, work with partners in education, social care, Early Help and voluntary sector to deliver services to local communities. As a staff nurse you will hold a caseload of children requiring targeted and specialist HCP level support. You will carry out holistic health assessments and plan care for children and young people.

Responsibilities
  • Work as part of a team to provide advice, care and support to children and young people, and their parents/carers in a locality of the borough
  • Assess, plan, implement and evaluate care for individual children and young people
  • Contribute to the planning and delivery of health promotion programmes and interventions
  • Work as part of multi-disciplinary and agency teams to support children, young people and families

  • Provide advice and guidance to parents/carers and professionals on a range of health and health-related issues
  • Support children and young people with continence and enuresis problems
  • Be able to produce comprehensive reports and undertake analysis of risk
  • Work with professionals from partnership agencies to provide localized support to children and young people
