Job summary

A social services agency is seeking an experienced Social Worker for the Adult Mental Health team in Stockton-on-Tees. Purpose of Role

Social Personnel is advertising a Social Worker role for Stockton‑on‑Tees Borough Council. We are seeking an experienced and motivated Social Worker to join the Adult Mental Health team. You will play a key role in safeguarding, supporting, and empowering adults with mental health needs, ensuring they receive personalised care and protection in line with statutory duties.

Key Responsibilities
  • Conduct strength‑based assessments under the Care Act 2014, delivering tailored support plans.
  • Work collaboratively with partner agencies to ensure coordinated and effective interventions.
  • Support individuals detained under the Mental Health Act 1983, ensuring safe discharge and robust Section 117 aftercare plans.
  • Assess eligibility for Continuing Health Care (CHC) and lead Decision Support Tool meetings.
  • Carry out mental capacity assessments and make best‑interest decisions under the Mental Capacity Act 2005.
  • Engage with the Court of Protection to uphold individuals' rights and safeguard wellbeing.
Essential Skills & Experience
  • Minimum of two years' experience working with adults whose primary need is mental health.
  • Qualified Social Worker registered with Social Work England.
  • Strong knowledge of relevant legislation including the Care Act 2014, Mental Health Act 1983, and Mental Capacity Act 2005.
  • Excellent communication, assessment, and multi‑agency working skills.
Desirable
  • Experience of working within local authority adult social care services.
  • Knowledge of local health and social care pathways across Stockton‑on‑Tees.
