Summary:
- Key Duties: Provision of Clinical Pharmacy Services:
- Provide clinical pharmacy services on rotation to the acute medical, surgical, emergency, and critical care units WRH, under the clinical supervision of Lead Specialist Pharmacists.
- Ensure compliance with medicines legislation, the Trust Medicines Policy, and medicines-related national and local guidelines.
- Plan duties, prioritize workload, and communicate effectively with medical and nursing staff.
- Interview newly admitted patients, review medication history, and construct pharmaceutical care plans.
- Monitor TPN, assist with therapeutic blood monitoring, and utilise MS Word, Excel, and PowerPoint for various tasks.
- Provision of Information and Counselling:
- Advise healthcare professionals on safe management, handling, and disposal of medicines.
- Assist in providing medicines information to consultants, General Practitioners, and other healthcare staff.
- Counsel patients with complex needs on their medications.
- Provision of Professional Dispensary Services:
- Scrutinize prescriptions for accuracy, appropriateness, legality, and cost-effectiveness.
- Question and challenge prescriptions when necessary in the best interest of the patient.
- Utilize Pharmacy Computer System and Electronic Discharge System for dispensing and supply of medicines.
- Provision of Cancer Services:
- Check chemotherapy prescriptions for accuracy and compliance with commissioning frameworks.
- Pre-check and release chemotherapy in aseptic units, ensuring compliance with GMP.
- Use Electronic prescribing system for managing chemotherapy patients.
- Managerial Responsibilities:
- Provide clinical supervision for pre-registration pharmacy graduates, pharmacy technicians, assistant pharmacy technicians, and students.
- Assist in maintaining the security of medicines and supplies within the Pharmacy Department.
- Financial Responsibilities:
- Conduct regular checks of Controlled Drugs on hospital wards.
- Provide advice and support on the Safe and Secure Handling of medicines.
- Provision of Education, Training and CPD:
- Participate in personal development review, agree on objectives, and demonstrate achievement through self-audit.
- Identify personal training and development needs, and complete yearly training plan in line with objectives.
- Other Operational Responsibilities:
- Assist in delivering pharmacy department’s dispensing and operational, and clinical services.
- Undertake emergency duties of a pharmaceutical nature as required.
