Job description

To provide high-quality, compassionate nursing care to patients with diverse neurological conditions, assisting them in their recovery and rehabilitation within the regional inpatient neurological rehabilitation service based at Mount Gould Hospital, Plymouth.

The post holder will utilise clinical skills to assess, plan, implement, and evaluate individualised care plans and goals tailored to meet the unique needs of each patient.

Responsibilities will include monitoring and managing neurological symptoms, administering medications, conducting health education, and providing support to patients and their families throughout the rehabilitation process.

This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

Main duties of the job

To assess capacity, gain valid informed consent, document appropriately and demonstrate the ability to work within a legal framework with patients who lack capacity to consent totreatment.

To comply with local confidentiality policies, child protection and vulnerable adult procedures.

Support patient care through provision of clinical knowledge, clinical reasoning skills,practical clinical nursing skills and assist in teaching knowledge and clinical skills to otherhealth care professionals.

Provide education and support to patients and their families on the nature of their neurological condition, treatment options, self-care strategies, coping mechanisms, and community resources available to support their recovery.

Maintain accurate and detailed documentation of assessments, care plans, interventions, progress notes, goal setting and outcomes to support continuity of care and adhering to Livewell Southwest policies and procedures

Participate in clinical audits and quality improvement initiatives to enhance the standard of care and promote evidence-based practice.

Advocate for patients' needs, rights, and preferences within the healthcare system to ensure they receive high-quality and person-centred care that promotes their well-being and recovery.

Participate in the education and assessment of performance of students and junior staff,in order to provide competent advice and clinical support whilst promoting the effectiveand efficient delivery of a quality service.

Job responsibilities

Responsibility for People Management:

Provide guidance and education to less experienced staff, including health care assistants and student nurses, ensuring they understand their role and responsibility with the neurological rehab unit. This will include the use of formal appraisal, line management, practice supervision and use of related documentation.

Participate in induction and support programmes in relation to new staff and volunteers.

Take responsibility for own personal and professional development, maintain competence,knowledge and skills commensurate with the role.

Coordinate and delegate tasks effectively to optimise patient care.

Be an active member of the in-service training program by the attendance and delivery of presentations and training sessions at staff meetings, tutorials, in-house training sessions and by attending external courses and practicing reflective practice to develop own and others skills, knowledge and expertise.

To assist in the recruitment of relevant grades of staff, as required.

Take a flexible approach to supporting colleagues in times of caseload pressure, including working with colleagues to maintain clinical cover across the team within the resources available. Foster a collaborative and supportive team environment by promoting a culture of open communication, mutual respect and effective teamwork amongst all staff members.

Responsibility for financial and/or physical resources:

The post holder will have no direct responsibility for finances; however, they will adhere to the relevant trust policies on financial issues.

Responsibility for administration:

To ensure competency in record keeping, maintaining an up-to-date record of all patient contacts, including MDT care plans, goals, assessments, observation charts and reports whilst always ensuring confidentiality in line with professional and organisational guidelines.

To report accidents, incidents or complaints following LSW procedures and policies and tocomply with the most recent Health and Safety at Work Act.

To attend and contribute to relevant team/service meetings.

To access and respond to work related emails and other information on a regular basisthrough Livewell Southwest intranet.

Responsibility for people who use our services:

Be an autonomous practitioner, working within appropriate professional guidelines and codeof conduct.

To undertake the comprehensive assessment of neurological inpatients, including those with a complex presentation, using investigative and analytical skills, to formulate individualised management, treatment plans and patient goals in collaboration with the MDT, using clinical reasoning and utilising a wide range of treatment skills and options.

To ensure a high standard of clinical care and support less experienced staff to do likewise.

Act as a patient advocate ensuring individual needs, preferences and choices are deliveredby the service.

Provide specialist advice via written or verbal instruction to patients, carers and relatives topromote understanding of the aims of rehabilitation, and to ensure a consistent approach topatient care. This may be health education, preventative or condition- specific advice.

Evaluate patient progress, reassess and liaise with MDT to discuss changes.

To have a flexible approach to competing priorities to meet the needs of the patient and theservice.

To work jointly with other health and social care colleagues and services (for example GPs,District Nursing, Social Services, private sector and therapy colleagues) to ensure patientsidentified rehabilitation and care needs are met through delivery of a coordinated multidisciplinary service. This will include goal setting meetings, progress meetings and dischargeplanning.

To comply with local confidentiality policies and be able to respond effectively to any concerns of abuse/safeguarding and know procedures they need to follow to escalate their concerns.

Comply with all infection control policies and guidance and attend relevant updates to support the needs of the service.

To ensure privacy, dignity and human rights are respected for all patients and carers.
