Social Worker – Hospital Discharge - Lincolnshire - £38ph

• Location: Pilgrim Hospital
• Hours: Monday–Friday, 37 hrs/week
• Rate: £38.00 per hour (Limited/Umbrella)
• On-site | No weekend working

Key Responsibilities

You’ll be based in a newly developed frailty ward supporting adults with short-stay admissions of up to 72 hours. Patients arrive both from the front door and the community, making this a varied, responsive, and hands‑on role.

As the ward’s dedicated Social Worker, you will:

• Manage a busy and complex caseload with confidence.
• Work as a fully integrated member of a Multi‑Disciplinary Team, collaborating with Consultants, Nurses, Therapists, Pharmacists, and wider support services.
• Use your expertise to support safe, timely, and person‑centered discharges.
• Apply strong knowledge of safeguarding, mental capacity, and Care Act compliance.
• Liaise across multiple hospitals, councils, and health trusts as required.
