Qualified Social Worker – Hospital Discharge
Location: Merseyside
Pay Rate: £46 per hour

We’re looking for an experienced Qualified Social Worker to join a dynamic and supportive Hospital Discharge Team in Merseyside. If you're confident in a hospital setting and passionate about delivering timely, person-centred support — this role is for you.

The Role

As part of this fast-paced hospital team, you’ll be working with adults and their families at key moments in their care journey — supporting safe, efficient hospital discharges and reducing delayed transfers of care. You’ll assess needs, coordinate services, and help individuals regain independence — all while navigating complex systems and collaborating with health and social care partners.

Key Responsibilities

• Complete timely assessments of adults in hospital and identify safe discharge pathways
• Work with families, hospital teams, and community providers to coordinate care plans
• Uphold best practice and legal frameworks (Care Act 2014, Mental Capacity Act, etc.)
• Use your professional judgement to manage risk, safeguarding, and capacity concerns
• Maintain clear, accurate, and strengths-based case recordings
• Promote independence, dignity, and person-centred planning

About You

• Qualified Social Worker, registered with Social Work England
• Minimum 2 years’ experience, ideally within hospital discharge or adult services
• Confident in managing hospital discharges, capacity assessments, and safeguarding
• Skilled at working in multi-disciplinary teams
• Calm under pressure, clear in communication, and solution-focused
