Role Purpose

To provide comprehensive primary care management and social work services for adults within a defined area, ensuring safe and timely hospital discharges while promoting wellbeing and independence.

What You Will Do

Conduct comprehensive assessments for adults, involving relevant individuals and agencies as required.

Design and implement care packages tailored to assessed needs within policy and budget frameworks.

Identify and coordinate resource providers to ensure effective care delivery.

Appoint key workers for ongoing monitoring and client support where appropriate.

Monitor and review care delivery, addressing issues with the Team Manager or provider.

Conduct timely re-assessments to determine ongoing support needs or service withdrawal.

Participate in team processes such as allocations, supervision, and appraisals.

Perform additional duties as required by management.

Person Specification

Professional qualification in Social Work and registration with Social Work England.

Experience in safeguarding, investigations, and multi-agency collaboration.

Strong knowledge of legislation including the Care Act 2014 and the Mental Capacity Act 2005.

Desirable: Practice Educator Award, BIA Assessor training.

IT proficiency for case recording and database management.
