Key Responsibilities
  • Care coordination of complex cases under the Care Programme Approach (CPA).
  • Developing co-produced care plans with service users.
  • Undertaking risk assessment and management.
  • Safeguarding adults and ensuring compliance with statutory duties.
  • Completing holistic assessments of mental health and social care needs.
  • Carrying out Care Act assessments and reviews.
  • Engaging with carers and wider support networks.
  • Partnership working with statutory and voluntary organisations.
  • Liaising with inpatient units to support discharge planning and continuity of care.
  • Maintaining accurate records and producing professional reports.
Essential Requirements
  • Degree/Masters in Social Work and current SWE registration.
  • Experience of working with adults with psychosis and complex needs.
  • Demonstrable experience of Care Act assessments and reviews.
  • Strong organisational skills with the ability to manage time effectively and prioritise workload.
  • Excellent interpersonal skills, with the ability to engage service users from diverse backgrounds.
  • Ability to work as part of a multidisciplinary team.
  • Good knowledge of relevant legislation, policy, and procedures, including Safeguarding, the Mental Health Act, the Care Act, and the Mental Capacity Act.
  • Right to work in the UK.
