Job description
Overview

We have an exciting opportunity to recruit an Intellectual disability nurse to our Primary care liaison Team. We are looking for a motivated and enthusiastic RNLD who is passionate about supporting reasonable adjustments for patients with intellectual disabilities in the community. We welcome applicants who are interested in supporting people with intellectual disability to enable them to have their physical health needs met within a primary care setting. We support practices to keep a learning disability register and promote the health needs of the population. We are looking for someone who is passionate about enabling our patients to access generic health screening and break down barriers in order to achieve this. The post holder will be committed to improving the life chances of people with intellectual disabilities.

We work with individuals to facilitate access to primary health care services and liaise with other health professionals within multi‑agency teams to advocate for service users. We work with local GP practices to identify patients who are eligible for an annual health check and encourage them to have the annual health check. We deliver training around the Enhanced Service for health checks to primary care colleagues. We work with national screening programmes to support them in meeting the needs of the local intellectual disability population and breaking down barriers to access to screening.
