Job description

The CHCP Wellbeing Primary Care Liaison Service is offering a fantastic opportunity for a candidate to join their team on a 12-month temporary/secondment basis as a Primary Care Liaison Nurse.

We are looking for exceptionally motivated, dynamic, and enthusiastic individuals with appropriate experience to join our team within City Health Care Partnership Community Interest Company (CHCP CIC) Wellbeing Primary Care Liaison Service.

The successful applicant must be flexible, a good team worker, and used to working under pressure. They will be required to have excellent interpersonal skills and be able to effectively communicate with patients.

Main duties of the job

Main duties of the role include:

  • Supporting the delivery of SMI and LD annual health check clinics with GP practices and acting as a source of specialist information/advice regarding annual health checks for people with Learning Disabilities and Serious Mental Illness.
  • Reviewing, analysing data, and collating reports to be produced for commissioners. This includes supporting the development and audit of identified QOF registers within GP practices across the city of Hull.
  • To engage with Primary Care Network's to promote and support the uptake of annual health checks. This will incorporate working with Service Leads in the innovation and development of projects to aid in reducing health inequalities.
  • To work in collaboration with secondary care service providers to implement and strengthen work on "integrated care pathways" and the interface between primary care and secondary care.

  • Dual trained nurse in two adult branch areas
Knowledge
  • Significant and demonstrable experience working in a Mental Health or Learning Disabilities setting
  • Use of IT equipment to produce reports and audits
  • Professional accountability
  • Previous experience of working with patients with mental health or learning disability problems
  • Knowledge of mental health risk assessment
  • Knowledge of psychotropic medication
