To provide a trauma-informed, person-centred domestic abuse support service embedded within GP practices across Calderdale. The role will support victims and perpetrators of domestic abuse identified by General Practice staff, facilitate rapid risk assessment and safety planning, and act as a bridge between primary care and specialist domestic abuse services.
This is a new service and the postholder will be required to set up the service, develop pathways and materials, build trusting relationships with partner agencies.
The post holder will act as a resource for members of the multidisciplinary primary care teams across Calderdale, facilitating an environment of learning to promote the development of staff. Working in the Inclusion Health team and in collaboration with safeguarding teams, secondary care, primary and community care providers to support the day-to-day service delivery to enable the provision of high quality, specialist support to patients who have experienced and perpetrated domestic abuse.
You will hold a small, short term caseload, providing pro-active, emotional, and practical and advocacy support to patients.
