A leading healthcare provider in Tees Valley is looking for an experienced Practice Nurse to work within Primary Care. The ideal candidate will manage a diverse caseload, addressing various health needs while ensuring high-quality care. Key qualifications include a nursing degree, current NMC registration, and at least three years of experience in a primary care setting. This position is full time but part time hours may be considered for the right candidate.

Experience of working in a primary care setting is essential and the applicant must have a minimum of 3 years experience of working autonomously.
In general, the post holder will be expected to undertake all of the normal duties and responsibilities associated with a Practice Nurse (Spirometry) working within Primary Care. The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients including;

YOU MUST HAVE:
Qualifications:
Diploma or Degree in Nursing
Evidence of practice in respiratory care
Current registration with NMC
Knowledge and Experience:
Recent respiratory care in either an acute orcommunity setting.
Spirometry procedure and interpretation.
Managing patients with complex and/or long-termconditions.
Understanding of Primary Care, including use ofsystems and processes, and particularly with regards to confidentiality.
A commitment to maintaining your clinical skillsand knowledge, to ensure our patients receive evidence-based care.
Excellent communication and problem-solvingskills
Excellent time management skills / ability towork to deadlines
A full Driving Licence and use of vehicle.
Post graduate course related to respiratory care.
Spirometry accredited course.
Pulmonary Rehabilitation course.

Key Responsibilities
Line manage and lead the HCA to ensure that the nursing team are cohesive and provide a team driven experience. Undertaking their objective setting and reviews.
Facilitates change in the health and wellbeing of patients, assisting patients to identify their health needs.
Works as part of the team to achieve good outcomes in; CQC, QOF, enhanced services and local improvement schemes
Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.
Ensure infection control guidelines are implemented and maintained, taking the practice lead for audits and updates, supported by the ELM Lead Nurse.
Ability to monitor and manage maintenance of stock and equipment to include refrigeration and emergency equipment.
Assessing the health care needs of patients and taking action
Leading the well person clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise.
Primary prevention and advice
The post-holder will participate fully in attaining QOF points and providing all services in the APMS Contract
Prescribing in accordance with the prescribing formulary (or generically) whenever this is clinically appropriate
Leads the routine immunisations programme including; flu, covid, shingles, travel vaccinations and others
Leads the childhood immunisations programme ensuring vaccinations given
Participation in the running of chronic disease management clinics.
Lead and deliver patient diagnostic and testing such as; ECG, Cytology, phlebotomy and others
Support and educate patients to adopt and promote healthy lifestyles and apply principles of self-care
Supporting the practice and PCN delivery of the Covid response / Vaccine delivery
