As a Practice Nurse & ICP Lead, you will be involved in a wide variety of essential nursing duties, working alongside a multidisciplinary team in a forward-thinking and patient-centred environment. We are especially interested in candidates with experience in compression bandaging and taking a lead in Infection Prevention Control.
Main duties of the job
- Chronic Disease ManagementExperience in managing chronic conditions, such as asthma, COPD, and diabetes, with an ability to provide patient-centred care plans and reviews.
- Womens Health ServicesIncluding cervical screening (smears) and contraceptive advice, in line with UK standards and guidelines.
- Childhood ImmunisationsProviding vaccinations and immunisation support, promoting a healthy start for children.
- Travel Health Advice and VaccinationsSupporting patients with essential travel health services, including vaccination administration and patient education on travel-related health.
- Health Promotion and Disease PreventionOffering lifestyle advice, smoking cessation support, and proactive health education to improve patients' well-being.
- Wound Care and ManagementDelivering assessment and management of wounds, including dressings, compression bandaging and suture removal, ensuring best-practice care and timely healing.
- Infection Control and Cold Chain ManagementTaking a lead in safe infection control practices and adherence to vaccination/ medication storage protocols.

Primary Responsibilities:
The following are the core responsibilities of the Practice Nurse. There may be occasions where other duties are required, depending on workload and staffing levels.
Clinical Care
- Assess, plan, and deliver evidence-based nursing care to meet patient needs.
- Manage and support patients with chronic conditions such as asthma, COPD, and diabetes, providing patient-centred reviews and care plans.
- Identify and support patients at risk of developing long-term conditions to help prevent deterioration.
- Provide routine nursing care, including cervical screening, in line with NICE guidance and national frameworks.
- Deliver wound care, compression bandaging and Doppler assessments as required.
- Provide and coordinate travel health advice and vaccination services.
- Request and process pathology investigations in accordance with clinical need.
- Administer and monitor adult and childhood vaccination programmes.
Record Keeping and Governance
- Maintain accurate, up-to-date clinical records using SNOMED CT coding.
- Ensure compliance with practice policies, including chaperone, infection control, and cold chain procedures.
- Support the team in maintaining clinical safety, including participation in emergency situations when required.
- Lead the practices Infection Prevention and Control (IPC) programme, ensuring policies, audits, and procedures are up to date, evidence-based, and compliant with national guidance.
- Provide training, support, and expert advice to the clinical and administrative team, promoting safe practices, monitoring infection risks, and driving continuous improvement in IPC standards.
Health Promotion and Patient Support
- Contribute to the development and delivery of health promotion and wellbeing programmes.
- Provide opportunistic health advice, lifestyle support, and smoking cessation interventions.
- Recognise and refer patients presenting with mental health needs appropriately.
- Liaise with external services and agencies to ensure appropriate support for vulnerable patients.
- Promote continuity of care and patient education through effective communication and follow-up.
Team and Service Development
- Support GPs and the wider clinical team as required.
- Participate in safeguarding processes in line with local and national policy.
- Contribute to quality improvement activities, audits, and shared learning.
- Work collaboratively with colleagues to enhance patient care and service efficiency.
Secondary Responsibilities
In addition to the core duties outlined above, the Practice Nurse may be required to:
- Participate in clinical audits and contribute to the evaluation and improvement of patient care.
- Develop, review, and implement nursing protocols and procedures as needed.
- Support and mentor colleagues within the nursing team, offering guidance and sharing expertise when appropriate.
- Contribute to local initiatives aimed at enhancing service delivery and patient outcomes.
- Participate in shared learning activities within the practice to promote continual improvement and professional development.
- Regularly review clinical practice, responding to changes in national policy, guidance, or best practice.
- Participate in the review and learning from significant and near-miss events to support a culture of safety and quality improvement.
