A healthcare provider in Wolverhampton is seeking a full-time Practice Nurse to join their team. The role requires developing health programs, creating individualized treatment plans, and providing routine nursing care. Candidates must possess a valid RGN registration and experience in primary care, particularly in managing chronic diseases. The ideal candidate will also have strong communication skills and be patient-focused, working within a collaborative multi-disciplinary team. We are currently looking for an experienced Practice Nurseto join our team at Ashmore Park Medical Centre. This is a full - time position(37.5hrs per week). We are looking for someone who is highly motivated,enthusiastic and committed to join our team.
Develop, implement, and embed health promotion and wellbeingprograms
Create and evaluate individual treatment plans for bothacute and non-acute conditions
Support the development of individual treatment plans forpatients with chronic diseases
Identify and manage patients at risk of long-termconditions, preventing adverse health effects
Provide routine nursing care in accordance withclinical-based evidence, NICE, and the National Standards Framework
Support patients in compliance with prescribed medications
Request pathology services as necessary and interpretresults
Maintain chronic disease registers
Prioritise health issues and intervene appropriately
Recognise, assess, and refer patients with mental health andcommunity health needs
Implement vaccination programs for adults and children
Liaise with external services/agencies
Support the clinical team with safeguarding matters,following local and national policies
Conduct a comprehensive physical health assessment ofpatients with complex multiple healthcare needs
Interpret results of different assessments to make diagnosisand plan to deliver care
Work independently or as part of a multi-disciplinary team
To lead on Infection Prevention Control
The practice has a very experienced, stable team, a single -handed GP, a part-time Advanced Clinical Practitioner, Health Care Assistant, aPractice Manager and a team of receptionists/administrators. We are alsosupported by various ARRS staff provided by the PCN to which we belong.
The practice is rated as 'good' by the CQC, is a high 'QOF'achiever.

Maintain accurate clinical records and personal care plans,using effective SNOMED codes
Maintain chronic disease registers
