The Lighthouse Group Practice is seeking a compassionate and dedicated Practice Nurse to join our healthcare team. The Practice Nurse will provide high-quality clinical care to patients of all ages, supporting the delivery of primary care services. This role involves conducting health assessments, administering vaccinations, managing chronic conditions, providing health education, and promoting preventive care. The successful candidate will work closely with GPs, other healthcare professionals, and administrative staff to ensure patient safety, efficient clinic operations, and positive patient experiences.
Main duties of the job
- Deliver evidence-based nursing care, including assessment, planning, implementation, and evaluation for patients with acute and long-term conditions
- Support chronic disease management, immunisations, wound care, cervical screening, travel health, minor surgery support, and health promotion
- Promote medicines optimisation, lifestyle advice, early identification of risk factors, and opportunistic prevention during patient contacts
- Maintain high standards of clinical governance, including accurate SNOMED CT / read coding, infection prevention and control, safeguarding, and emergency response
- Work collaboratively within the multidisciplinary team, contributing to audits, service development, training, and quality improvement initiatives
- Refer appropriately to secondary care and community services to ensure coordinated, holistic patient care

Job responsibilities
Clinical Responsibilities
- Assess, plan, implement, and evaluate patient care using evidence-based practice
- Manage acute and chronic conditions within scope of practice
- Deliver chronic disease management clinics (e.g. diabetes, asthma, COPD, hypertension, CHD)
- Provide wound care, including leg ulcer management and Doppler assessments
- Administer immunisations and vaccinations in line with national schedules
- Deliver cervical screening (where trained and accredited)Provide travel health advice and vaccinations
- Support minor surgery procedures and post‑operative care
- Request pathology investigations and interpret results in line with protocols
- Review and support patient medication compliance and concordance
- Provide lifestyle advice and health promotion (smoking cessation, weight management, alcohol advice)
- Recognise and respond to clinical emergencies
- Maintain accurate and timely clinical records using SNOMED / read coding
- Ensure compliance with infection prevention and control procedures
- Safeguard adults and children in accordance with local and national policies
- Use and maintain disease registers effectively
- Refer patients appropriately to secondary care and community services
- Deliver opportunistic health promotion during patient contacts
- Identify and support patients at risk of developing long‑term conditions
Infection Prevention, Safeguarding, and Governance
- Support the Infection Prevention and Control (IPC) Lead, ensuring compliance through audits, training, and reporting
- Contribute to the IPC annual statement in conjunction with the Lead Practice Nurse / Business Manager
- Ensure compliance with safeguarding, chaperone, and substance misuse policies
- Maintain accurate clinical records using SNOMED CT coding and ensure effective use of disease registers
Teamwork and Service Development
- Work cohesively as part of the nursing team, maintaining high standards of safe practice
- Contribute to audits, protocol development, significant event reviews, and quality improvement initiatives
- Participate in shared learning, training, and appraisal processes
- Liaise with external services and agencies to ensure holistic patient care
Working Conditions

Person Specification
Qualifications
- Registered General Nurse (RGN) with current NMC registration
- Up-to-date clinical training relevant to general practice.
Experience
- Experience working as a Practice Nurse in a primary care setting
