We are recruiting for a Practice Nurse to join our friendly team at Andaman Surgery.

This post has become available due to the retirement of one of our practice nurses.

You should have experience working in General Practice.

Main duties of the job
  • Seeing Patients face to face, providing clinical assessment and care.

Andaman Surgery is a 4 Partner practice. we are a small friendly and very effective team.

The nursing team consists of 3 practice nurses 2 HCAs, a TNA, and a Nurse Practioner.

we hold regular education meetings and activley encourage ongoing training.

Job responsibilities

ROLE / RESPONSIBILITIES

Clinical practice:To maintain accurate contemporaneous computerised clinical records of every contact using templates for NICE Guidelines and Prescribing information. including history, examination, plan of care, advice given, and recommended follow-up. This includes documenting adverse events, safeguarding child or vulnerable adult issues, or other notifiable matters.

To see patients, face to face for known long-term conditions including Diabetes, COPD, Asthma, and Hypertension. Assess, plan, implement, and evaluate individual treatment plans for these patients.Perform a holistic assessment of patients attending for cervical cytology smear tests.

Provide a holistic approach to wound management and implement wound care in line with current evidence-based guidelines.Suture removal.Proactively identify and manage treatment plans for patients at risk of developing a long-term condition as appropriate.Work with other health care professionals to, monitor, manage, and treat long-term conditions, including non-drug-based treatment methods using a management plan, in line with national and local policies, and to refer as needed.

Work with patients to support adherence to prescribed treatments.Work within patient group directions/patient-specific directions in collaboration with GPs and other primary care nurses to meet the needs of patients.Support patients in adopting health promotion strategies that encourage them to live healthily and apply principles of self-care.

Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedures and local guidance.To be aware of and work within the performance framework for the Care Quality Commission and the National Quality Requirements.EducationalContinue to develop, expand and role model own personal clinical expertise.To contribute to the development of the service as a learning organisation.

Awareness of national standards of infection control and regulatory/contractual/professional requirements to work within good practice guidelines.

To contribute to the development of clinical services within the serviceCommunication

Be aware of responsibilities under the Data Protection Act.Ensure effective communication takes place across teams using face-to-face, and online consultations.Recognise patients needs for alternative methods of communication and respond accordingly.To liaise with nursing colleagues and other professionals.

Personnel:To be flexible and responsive to the clinical needs of patients and the operational needs of the service.Take responsibility for maintaining a record of personal and professional development.
