We're thrilled to announce an exciting opportunity for a Practice Nurse to join our growing team.

As an advanced training practice, we proudly support a diverse range of allied health professionals and work closely with our Primary Care Network (PCN) to provide comprehensive patient care. We're looking for a dedicated Practice Nurse who is passionate about delivering high quality nursing care, collaborating with our healthcare team, and supporting the delivery of policies and procedures. Main duties of the job

Key Responsibilities:

  • Patient Care: Deliver general practice nursing care, including chronic disease management, immunisations, wound care, and health screenings. Occasionally undertake home visits as needed.
  • Health Promotion: Educate and advise patients on health promotion, disease prevention, and self-management strategies for long-term conditions.
  • Clinical Procedures: Perform clinical procedures such as venepuncture, ECGs, and cervical screening, ensuring adherence to clinical guidelines.
  • Care Planning: Develop, implement, and evaluate individualised care plans for patients, ensuring a patient-centred approach.
  • Team Collaboration: Work as part of a multidisciplinary team, liaising with GPs, healthcare assistants, and other healthcare professionals to ensure comprehensive patient care.
  • Record Keeping: Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with data protection regulations.
  • Professional Development: Engage in continuous professional development, attending training sessions and keeping abreast of current nursing practices and guidelines.
