A forward-thinking GP practice in Greater London is seeking a flexible Practice Nurse to join their team caring for 16,000 patients. You will provide a full range of nursing tasks, participating in chronic disease management, health screenings, and patient education. The ideal candidate will have good nursing skills and experience in minor illness management, with ongoing professional development. Training can be arranged for those moving from another sector. We are a forward thinking trainingpractice in a diverse area, which is open, green and close to London. We lookafter 16,000 patients and offer a full range of primary care services, based ina beautiful new surgery. Our dynamic team compromises of two nurses, one ANPand an HCA (currently recruiting another). We have room for another PracticeNurse, whether experienced or moving from another sector
(in which case training would bearranged).
You will provide the full range ofnursing tasks (subject to training) in what is always a varied and interestingjob. You will be flexible and willing to adapt in the fast moving healthservice. You will be a conscious learner, keen to learn new skills. You will bea good team player, supportive of your colleagues
(who will be supportive of you). Youwill have good IT skills

Providing assessment, screening and treatment services and health education advice
Help organise and co-ordinate the provision of nursing services for the practice as part of the nursing team
Provide nursing treatments and procedures to patients in participation with the GPs or independently to agree protocols
Participate in Chronic Disease management to practice protocols, referring to or liaising with the patients usual GP as needed
Provide general and specific health screenings and assessments to the practice patients within agreed protocols liaising with the GPs as necessary concerning risk factors or other items identified
Triage patients and treat those with minor illness as needed
Adviseand educate patients on general health care with referral to or liaison withGPs as necessary
Maintaina collaborative, supportive and holistic approach to patient care at all time
Arrange for and perform appropriate tests as indicated such as blood, swabs etc.
Make referrals as necessary to other health and social care professionals

To be an autonomous professional working to the highest standards and an integral member for the practice team caring for patients registered with the practice, carrying out duties within the role of Practice Nurse to include diagnosis, examination and treatment of patients, preventative care, screening and patient education.
All work to be carried our within your areas of competence and within the practice policies and procedures.
