A healthcare center in Bridgnorth is seeking a compassionate and proactive Practice Nurse to join their dedicated nursing team. The role is part-time, requiring 16 hours over two fixed days. You will engage in patient management, health promotion, and support clinical emergencies while collaborating with a diverse team of healthcare professionals. Ideal candidates will have a Registered Nurse qualification and experience in a primary care setting, along with excellent communication skills and a commitment to ongoing professional development.
We are looking for a proactive, compassionate Practice Nurse to join our Nursing team. You will work collaboratively with HCAs, Phlebotomists, Nurse Practitioners, Registrars, Salaried GPs, GP Partners, and our well-established, supportive administration team.
This part-time role is for 16 hours across two fixed working days per week - Wednesday, including extended access, and Thursday.
Our practice is situated in a charming market town, featuring a picturesque town park with views of the River Severn, an ideal spot for a relaxing walk or lunch break!
We are a team made up of 9 GP Partners (male and female) 6 Salaried GPs (male and female), 2 Nurse Practitioners, 4 Nurses, 5 HCAs and 4 Phlebotomists, working across two rural sites. We are a training practice with several Registrars and Medical Students.
We have a large, knowledgeable administration team, and an excellent team spirit throughout.
The Team is dedicated to providing an excellent service to our patients whilst promoting healthy living.
We are part of a Primary Care Network consisting of 9 practices. We meet regularly and support each other clinically and non-clinically.
Manage caseload of patients onthe clinical system across sites
Management of patients includingmonitoring (BP, ECG) and vaccination programmes
Support the team in dealing withclinical emergencies
Support the teams withsafeguarding matters
Knowledge of chronic diseases(asthma, COPD, diabetes, HF, etc.)
Good working knowledge andexperience incomplex dressings and supporting pathways
Opportunistic health promotion(diet, smoking, alcohol, exercise)
Ensure Nurse Registration,revalidation and indemnity are up to date
Input accurate and timelyinformation into patients EHR using approved coding
Work safely within IPC,undertaking audits, aseptic techniques when required
Initiate and participate inclinical audits
Actively participate insignificant even analysis (SEA)
Maintain appropriate training,skills and knowledge
Contribute towards achievingaudit targets for the Practice
Participate in management ofrecording of medical supplies and stock expiry dates
Maintain testing and recordingof treatment room equipment, reporting any malfunctions to management
Chaperoning of patients andclinicians as required
Adhere to all relevant policiesincluding IPC and cold chain management
Use of PPE and launderingprovided uniform as per guidance
Apply spill control anddecontamination policies
Demonstrate good practice forequal opportunities, culture and diversity
Any other appropriate duties asrequested by Management or Partners
