Physical Therapist - Per Diem-Hourly

For over 40 years, North Hill, a not-for-profit senior living community located in Needham, Massachusetts, has provided vibrant living opportunities to people 65+ by offering a host of hospitality, wellness and health care services. The Physical Therapist is responsible for providing evaluation and treatment to patients, functioning as a member of the interdisciplinary team, providing patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation.

Job Specific Duties

This is an hourly role not per visit.

  • Knowledge of Medicare Conditions of Participation as well as State and Federal Regulations
  • Provides patient care and documents treatments in accordance with physicians’ orders, agency policy, and patient needs
  • Performs patient evaluations, selecting appropriate diagnostic material to assess all aspects of care informing the physician of recommended interventions
  • Develops a care plan in conjunction with patient, identifying patient centered goals establishing priorities of care
  • Promotes professional growth by attending, presenting formal and informal educational experiences, in-services, conferences, and seminars to team
  • This position also assists in the supervision of assistants, Certified Home Health Aide (CHHA), students and interns as assigned according to Medicare Condition of Participation
  • Other duties include scheduling patients and staff, participation in discipline specific projects i.e. chart audits, QI data collection, peer review program, student program and Certified Home Health Aide training. This position provides direct patient care
  • Requires participation in interdisciplinary care conference and completion of visit documentation within 24 hours
  • Follows all Emergency Management Plan and actively participates in fire and evacuation drills
  • Meets department uniform, appearance and grooming requirements
  • Complies with all OSHA safety regulations, local applicable laws regarding health, safety or environment
Required Education and Experience

Bachelor of Science in Physical Therapy

  • Current Professional License within therapy discipline in the Commonwealth of Massachusetts.
  • Must have a valid MA driver’s license and an automobile available to conduct community home visits.
  • Current CPR/AED certification.
  • 2 years' experience as a Physical Therapist in a professional setting
  • Proficient with computer software programs
Preferred Education and Experience
  • Must be able to communicate effectively with Residents, Team Members and Visitors and must be able to communicate effectively in English.
  • Must be able to function effectively in an organization undergoing change in the model in which patient care is delivered and to support the changes in the model.
  • Willingness to participate in the change process by working with the team to problem solve and develop systems to support the highest possible quality care and services to Residents.

Pay Range: $60/hour Individual compensation within this range is based on a candidate’s specific qualifications, including education, overall experience, relevant experience, and specific skills.
