Job Description

Summary

The Home Health Physical Therapist Assistant (PTA) performs treatment, provides exercise instruction, and patient education aimed at improving the patient's well-being while following the established plan of care.

Responsibilities

  1. Under supervision of a Physical Therapist, follow all state laws governing physical therapy in home care, adhere to the treatment plan set by the Physical Therapist, and perform only procedures qualified and trained for.
  2. Follow the plan of care, instruct and assist patients within the scope of practice and state laws.
  3. Observe, record, and report patient responses and condition changes to the supervising PT, nurse, and/or physician. Coordinate care with healthcare team members as appropriate.
  4. Instruct patients, families, caregivers, and other healthcare team members in therapy within the scope of the physical therapy assistant.
  5. Visit patients within the service area and nearby agencies as needed.
  6. Complete and submit progress notes for each visit according to policy.
  7. Participate in staff meetings and committees as requested.
  8. Adhere to LHC Therapy Standards of Practice.
  9. Participate in weekend rotations as required.
  10. Perform all other duties as assigned.
Education and Experience

License Requirements

  • Current license, registration, or certification to assist in physical therapy practice in Florida.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle.
