Job Description
Summary

The Home Health Physical Therapist Assistant (PTA) performs treatments, provides exercise instruction, and patient education aimed at improving or enhancing the patient's well-being, following the established plan of care.

Responsibilities
  1. Follow all state-specific laws governing physical therapy in home care, under the supervision of a Physical Therapist.
  2. Perform only procedures qualified and trained for, and recommend changes to the plan of care to the supervising PT.
  3. Instruct and assist patients in evidence-based treatments within the PTA scope and legal boundaries.
  4. Observe, record, and report patient responses and condition changes to the supervising PT, nurse, and/or physician.
  5. Coordinate care with healthcare team members as appropriate.
  6. Educate patients, families, caregivers, and healthcare team members about therapy within the PTA scope.
  7. Visit patients across the service area and nearby agencies as needed.
  8. Complete and submit progress notes for each visit as per policy.
  9. Participate in staff conferences and committees as requested.
  10. Adhere to LHC Therapy Standards of Practice.
  11. Participate in weekend rotations to provide services as needed.
  12. Perform other duties as assigned.
Education and Experience
License Requirements
  • Current license, registration, or certification to assist in physical therapy practice in Florida.
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, and access to a dependable vehicle.
